Heidi 4: Mösleins Bergwelt (1992)

Heidi’s the only named performer, slim brunette in her 20s with dark curly hair, wearing heels and a dress that gets tossed aside quick. The scene starts with her kissing a boot in close-up, then jumps straight into the action with two muscular black dudes in a dim medical office setup — shelves with equipment, a chair, candles flickering on the walls.
